This page helps FormChase users and people whose data may be processed in FormChase exercise their privacy rights. It complements the controls available inside the app and the full Privacy Policy.
1. Who can use this page
- People with a FormChase account.
- People who no longer have app access but want to request account or data deletion.
- People whose information was added by a Professional User, such as a trainer or nutritionist using FormChase with their clients.
- People who contacted FormChase support or sent a bug report.
2. What you can request
Access or export
Ask for a copy of the personal data associated with your FormChase account, including account, workout, nutrition, body measurement and support data where applicable.
Correction
Ask us to correct inaccurate account data. Most profile, workout and nutrition records can also be edited directly in the app.
Deletion
Delete your account and associated personal data, or request deletion of specific data where deletion of the whole account is not needed.
Restriction or objection
Ask us to restrict processing, object to certain processing, or withdraw consent for optional processing such as analytics or data sharing with a Professional User.
3. Fastest way to submit a request
- Open FormChase and sign in.
- Go to Settings → Privacy & Data.
- Use the built-in controls to export data, manage consent, view previous requests, or delete your account.
The in-app route is usually the fastest because it verifies your identity through your signed-in account.
4. Submit without app access
If you cannot access the app, email contact@formchase.com from the email address linked to your FormChase account. Use one of these subjects:
- Delete my account
- Export my FormChase data
- Correct my FormChase data
- Privacy request - FormChase
Include the account email, the request type, and enough detail for us to understand the request. Do not send your password or unnecessary health information by email. We may ask follow-up questions to verify your identity before completing the request.
5. How we prevent unauthorised requests
Someone knowing your email address is not enough for us to delete, export or disclose your data.
- Signed-in app requests: high-risk actions such as export and account deletion require an authenticated session, fresh step-up verification, and final confirmation in the app.
- Email requests: we verify control of the account email before processing. Verification links are one-time, expire, and are rate-limited.
- Anti-enumeration: for logged-out requests, we use neutral wording such as "if an account exists" and do not confirm whether an email address belongs to a FormChase account until identity is verified.
- Different sender email: if a request comes from an address that is not the account email, we require additional evidence and normally ask the person to recover access or submit the request from inside the signed-in app.
- Requests on behalf of another person: we do not act on requests from a trainer, friend, employer or relative unless they prove legal authority and we can verify the data subject where appropriate.
- Audit trail: verified requests, decisions, and deletion actions are logged so we can demonstrate how the request was handled.
For Romanian users, we normally avoid asking for a copy of an identity document or CNP because that would collect extra sensitive information. If identity cannot be verified by account access, email control or store/account evidence, we will ask only for the minimum additional information needed to confirm identity.
If you receive a FormChase verification email or deletion-related message that you did not request, do not click the link and contact us immediately at contact@formchase.com.
6. Account and data deletion
For account deletion, use the dedicated Delete Account and Data page. It explains the in-app deletion path, the email fallback, what is deleted, what may be retained for legal reasons, and why an App Store or Google Play subscription must be cancelled separately in the store account.
7. View past requests
If you submitted a request in the app, open Settings → Privacy & Data to view its current status. If you submitted by email, reply to the confirmation thread so we can keep the request history together.
8. Response time
We respond without undue delay and in any event within one month of receiving a valid request, as required by GDPR Article 12(3). If a request is complex or we receive many requests, we may extend the deadline by up to two further months and will tell you why within the first month.
Requests are free of charge unless they are manifestly unfounded, excessive or repetitive, in which case GDPR allows a reasonable administrative fee or refusal.
9. Professional User data
If a trainer, nutritionist or other Professional User entered your data in FormChase, FormChase can process requests for data stored inside the app. That Professional User may also be an independent controller for copies or notes they keep outside FormChase. You can still contact us first and we will help identify which part is handled by FormChase and which part should be directed to the Professional User.

You may lodge a complaint with the Romanian supervisory authority, ANSPDCP (dataprotection.ro), or with the supervisory authority in your EU Member State of habitual residence.
